This form has some tricky questions, not to mention that Russian ladies with limited English will more than likely have problems understanding it.

That`s why I wrote an explanation guide for FSU ladies in Russian language:
- 1st page has some explanations on how to download Acrobat Reader and explanations how to fill out its tricky parts;
- 2nd is just an example of a filled form;
- 3rd is an example of a filled form with Russian headings so your Russian lady can understand what kind of information she is asked to provide.
